Chronic care management (CCM)
Comprehensive individualized care plan for people who have 2 or more chronic conditions (i.e. Hypertension, Diabetes, Hyperlipidemia)

How often can you be seen?Each patient is seen based on the Nurse practitioner’s assessment and plan of care. If medically necessary, you can be seen on a monthly basis.

When are co-pays and fees due?Please note that co-pays, past due fees, and service costs are required at the time of your visit. As a cashless business, we accept electronic payments only, which can be made via credit or debit card.

What to expect during a house call visit?You will meet with the nurse practitioner who will provide health-related services including exams, order necessary testing, prescribe medications and much more, all done within the comfort of your home.

What areas do we serve?We currently serve all areas in Hillsborough and Pinellas County.

Can I still see my regular doctor?Yes, we can act as a secondary provider for urgent needs but most will switch once they see the convenience.

What insurances do we accept?We currently accept Medicaid and Medicare Part B. We continue to work on partnering with other major insurance companies. Additionally, we offer affordable private pay options.

Do we provide home health services?No. However, we have the capability to order home health services and offer continuous monitoring of your care from start to finish.

Are there any travel fees associated with our services?Yes, we charge a flat rate of $20 for home visits greater than 5miles from our address.

What are the hours that the Nurse Practitioner visits?We see patients on Mondays and Fridays from 8:30 am to 4:30 pm. After-hour visits are available from 5:30 pm 8:30 pm on weekdays via Telemedicine at no extra cost
